Abstract

Remote Patient Monitoring (RPM) has emerged as a transformative healthcare innovation that enables continuous observation of patients outside traditional clinical settings through wearable devices, mobile applications, biosensors, and digital health platforms. The increasing prevalence of chronic diseases, aging populations, and rising healthcare costs have accelerated the adoption of RPM systems across healthcare organizations worldwide. RPM allows healthcare professionals to collect and analyze patient health data in real time, facilitating proactive interventions and personalized care management. Healthcare outcomes, including patient health status, treatment adherence, disease management, patient satisfaction, and quality of care, are critical indicators of healthcare effectiveness. Although RPM has demonstrated considerable potential in improving healthcare outcomes, the mechanisms through which these improvements occur require further investigation. Continuous Data Tracking refers to the ongoing collection and monitoring of patient health information through digital technologies, while Data Flow Optimization refers to the efficient transmission, integration, accessibility, and utilization of healthcare data across clinical systems. Continuous monitoring enables early detection of health changes and supports timely clinical interventions, whereas optimized data flow ensures accurate and efficient information exchange among healthcare providers. This study investigates the relationship between Remote Patient Monitoring and Healthcare Outcomes while examining the mediating roles of Continuous Data Tracking and Data Flow Optimization. Drawing upon Healthcare Information Systems Theory and Continuity of Care Theory, the study proposes that RPM positively influences Healthcare Outcomes through Continuous Data Tracking and Data Flow Optimization. A quantitative research design was employed using survey data collected from healthcare professionals, telehealth specialists, healthcare administrators, nurses, physicians, and patients utilizing RPM technologies. Data were analyzed using Partial Least Squares Structural Equation Modeling (PLS SEM) through SmartPLS software. The findings reveal that Remote Patient Monitoring significantly improves Continuous Data Tracking, Data Flow Optimization, and Healthcare Outcomes. Furthermore, both Continuous Data Tracking and Data Flow Optimization significantly mediate the relationship between RPM and Healthcare Outcomes. The study contributes to healthcare informatics, telemedicine, and digital health literature by identifying key mechanisms through which RPM enhances patient care and healthcare performance.
